Intake Questionnaire for Employment Matters
You can either download and print out the form or click here to fill it in online
Explanation of our procedures and charges:
Before we can provide you with any legal opinion or advice concerning your employment issue, we must have a full understanding of the factual circumstances involved. Therefore, it is imperative that you fill this questionnaire out as completely and accurately as possible.
The Questionnaire should be completed before your consultation, as it enables us to gather much more information than would otherwise be possible during our meeting with you. In addition, you should bring any and all documents concerning your employment with you for the attorney to review during the consultation.
Our Firm charges a flat fee of $175.00 for initial consultations in employment matters, payable prior to your meeting with the attorney. The consultation may take anywhere from 20 minutes to an hour. You will be advised as to whether the attorney believes you have a viable claim worthy of pursuing based upon the information you provide in the Questionnaire as well as during the meeting.
If you do not wish to have an in person consultation, you can choose to have the attorney review your completed Questionnaire for a reduced fee of $75.00. You must pay the fee before the attorney reviews the information and may do so using Pay Pal on our website. The attorney will then e-mail you regarding whether or not the attorney believes you have a viable claim worthy of pursing based solely on the information your provide in the Questionnaire. If the attorney says you may have a claim, you will then be offered an in-person consultation to discuss the matter at no additional charge.
The purpose of the consultation is to determine whether you have a potential claim or not. The Firm does not agree to represent you by consulting with you. If the Firm does offer to represent you, you will be required to sign a contract authorizing the Firm to represent you and agreeing to pay the Firm for legal services.
Please Note:
There are very strict statutes of limitations for legal matters, some of which are very short in duration. You must pursue any claims you might have in a timely manner or you will lose the right to pursue them forever.
Please complete the following information as completely and accurately as possible so that we may provide you with the best possible legal opinion regarding your situation:
